booking yourself online:
If you have insurance and you are not sure of your benefits, please contact me BEFORE you book yourself. I like to have at least 24 hours to check benefits because some insurance companies are easier to deal with than others. Please note that I am a provider for Cigna, Pacific Source, and Regence BCBS. If you have MODA I won't be able to bill as an in-network provider. If you have out-of-network benefits I will be able to bill your insurance company. It is up to you to find out if you have out-of-network benefits. Any claims that are denied are the responsibility of the client to pay in full.
If you have any questions please do not hesitate to contact me.
If you have insurance and you KNOW you have benefits, please go ahead and book yourself online (if you prefer you can also contact me to set up your appointment, whichever way works best for you) and follow up with an email providing the following information:
1. Name as it appears on your insurance card; 2. Your address 3. Your insurance ID#; 4. DOB; 5. Group # (not all cards will have this); 6. Provider phone # (on the back of your card; not all cards will have this). You may also take a photo of the front and back of your card and text/email it to me.
Please Note: My scheduling software will not allow new clients to book more than one appointment fort their first time making an online appointment. If you are new and wish to book multiple appointments before your first visit, please contact me to let me know and I can book them for you. I do this to prevent anyone from making multiple bookings before I can determine whether or not they have massage benefits. Thank you for your understanding!
The general rule is: please cancel/reschedule your appointment with no less than 24-hours' notice.
Exceptions are emergencies: Lives are complicated. You get into a car crash, your dog/cat/canary dies, your kid gets hurt, "snow" delays/cancels school, etc. Emergencies, being unavoidable, are not penalized.
On the other hand, if you no show because you forgot (email reminders are sent out, and "I didn't look at it" doesn't clear you of your responsibility to show up) or lost track of time, or delayed rescheduling, the penalty is waived the first time, but is assessed at $80 for all further missed/no-show/late cancel appointments after that. This applies to all clients regardless of whether they have insurance. If you have a $20 copay and you no-show/late cancel for a second time, the fine is $80, which is my cash discount price for an hour massage. If you no-show more than once for a 90-minute or 120-minute massage, the fine is the cost of that duration of massage. I do understand that having children makes scheduling difficult but please make the utmost effort not to reschedule the day of your massage multiple times in a row. Once or twice is okay but rescheduling the day of multiple times becomes problematic.
Parking in NW Portland, as in many neighborhoods around the city, is very tricky. You can find free hour or four-hour parking but it can be competitive and metered parking can be difficult to find as well. Please allow extra time to find a spot if you are driving.
My office is located inside an old converted powder blue house on the corner of NW 20th & Overton. There is a sign on the corner that reads "Overton Center." Suite 2 is on the first floor. Please come in and have a seat and I will be out to get you. There are several seating options and you are welcome to choose your preference. Help yourself to tea and water while you wait. The restroom is located on the first floor: you walk in, walk through the lobby and living room, turn left, and it will be straight ahead.
